  4. What will change in adult practice.

  6. Success criteria and decision rules.

  7. Your monitoring plan and review dates.

How To Write It Simply

Use short sentences. Use plain labels. Keep it to one page.

Create a one page document. Title it:
Week 4 action logic model – [School/Campus] – [Your name]

Your page meets expectations when all are true:

  • Action clearly fits the hypothesis.

  • Adult practice change is explicit, not vague.

  • Success criteria are measurable and time-bound.

  • Decision rules are clear and realistic.

  • Monitoring plan includes dates and owners.
